Residents are encouraged to fill out the form available at https://phoenix-gov.formstack.com/forms/animal_neglect. Once submitted, the report is directed to the appropriate agency, which may include the City of Phoenix Neighborhood Services, Maricopa County Animal Control, the Arizona Humane Society, or the Phoenix Police Department.
